Out of Network (PPO) Coverage
If you have a non-HMO insurance plan, you likely have out-of-network or PPO benefits. For out-of-network services, clients are responsible for the full self-pay rate at the time of service.
As a courtesy, we provide a superbill upon request. Clients may submit this superbill directly to their insurance carrier for possible reimbursement.
Reimbursement is not guaranteed and is determined solely by your individual insurance plan. It is the client’s responsibility to:
Confirm out-of-network mental health benefits
Verify deductible and reimbursement rates
Submit claims to insurance in a timely manner
Support with Insurance Verification
If you would like help preparing to contact your insurance company, please reach out to our office. We are happy to email you a call script to use when speaking with your carrier to confirm out-of-network reimbursement.
We will also provide the National Provider Identifier (NPI) for your assigned clinician, which your insurance company will require when processing your claim.

CANCELLATION POLICY
Appointments are considered LATE cancellations if the cancellation or rescheduling request is received less than 24 HOURS prior to the start time.
Late cancellations and no-shows will be subject to a $40 charge.
This is in consideration of the fact that we reserve one hour for each appointment. Thank you.
